– the reasons why you couldn’t be there.
– Tell him/her how you tried contacting him/her.
– Request another meeting.

Greetings! With profound regret, this letter is exclusively written to seek an apology for inability to attend meeting scheduled for 10th October. It is an imperative meeting for my organisation to finalise the deal with your company.
Unfortunately, my father falls sick, and he has been admitted to hospital. I am bringing the only child of my parents, and therefore, obliged to look after the wellbeing of my parents. Therefore, I have cancelled all appointments of the next ten days, so that I can take care of my father.
However, I tried to make a phone call, but you were busy in meeting, and your secretary was informed about my situation and also requested to arrange a phone call. On waiting for two days to call back, I decided to inform you via this letter. Therefore, it is a humble request to reschedule meeting at your convenience after 15th October. I shall be thankful for this kindness.
